Quarterly Report

For Executive Branch and MNSCU agencies with more than 25 employees

Each agency with more than 25 employees is required to submit a quarterly report of all the Unlimited, Classified hires filled through the Multi-Source process and the Non-Competitive, Qualifying hires in a job group (or subdivision) where a disparity exists Personnel Rule 3905.0700.

The hires are divided into the following:

  • Affirmative appointments
  • Non-affirmative appointments
  • Justified appointments
  • Non-justified appointments (missed opportunities)

SECTION ONE
Agency name

Year of Reporting  - Indicate the year this report is being submitted

Quarterly Reporting Schedule  - The report being submitted covers the following period


Completing the Form
 - Did your agency fill any unlimited, classified hires through the Multi-Source process where a disparity exists OR any non-competitive, qualifying hires in a job group where a disparity exists for the period being reported?

If Yes, complete the entire form. If No, proceed to Section Three (Agency Personnel).
