For most programs appeal requests must be written. They can be submitted by mail, fax, online, or in person:

  • To apply online, fill in and submit the online appeal form
  • Bring the written appeal to the Appeals Division at 444 Lafayette Rd. N., St. Paul, MN

Statewide

Conducts administrative fair hearings for appeals arising out of the broad array of programs administered by the Department of Human Services (DHS) These include:
  • Cash and food benefit programs
  • Health care programs
  • Social services programs
  • Residential programs
  • And others
Human services judges conduct hearings involving:
  • Allegations of program fraud
  • Maltreatment determinations made by DHS or local county agencies
  • Individuals who were disqualified from working in DHS-licensed facilities
  • Eligibility appeals for the MNsure insurance exchange
  • Child and vulnerable adult maltreatment
  • Disqualification hearings for the Minnesota departments of Health and Education
