Eligibility

Individuals must meet the following criteria:
  • 21 years or older if they have Medical Assistance, or 19 years or older if they have MinnesotaCare.
  • Experiencing homelessness or are unhoused.
  • In need of short-term medical care for fewer than 60 days
Program participants must also be ineligible for hospital-level care, skilled nursing care, or do not meet the severity of illness for other levels of care.

The main objective is to prevent re-hospitalizations and provide medical care for those who are unhoused, in a shelter, or otherwise lacking stable housing. Offers behavioral health support, care coordination, housing solutions, and a comprehensive range of social services to ensure the successful recovery of the patient
