Provides intensive case management to adults with mental illness who need extra support when their symptoms are at an elevated level. Team members are available to meet with individuals as many as four times a week for typically 3 to 6 months. The client and the team mutually agree upon individualized treatment goals to prevent or shorten hospital stays and avoid long-term hospitalization.
