Eligibility
You are eligible for Relocation Service Coordination - Targeted Case Management (RSC-TCM) if you meet all of these criteria:
- You choose to move to a community setting
- You have Medical Assistance when you receive RSC-TCM
- You live in an eligible institution, such as a nursing facility, regional treatment center, hospital or intermediate care facility for persons with developmental disabilities (ICF/DD)
- You are closed to any home and community-based services (HCBS) waivers.

Case management service to help people who want to move out of certain institutions into the community. Helps people plan and arrange services and supports to live in the community.
Services can include:
- Communicate with you and other necessary people to carry out the plan
- Coordinate with the institution discharge planner
- Develop and review a relocation plan with you
- Make referrals to needed services and supports
- Travel and complete documentation that is necessary to develop and carry out the plan
