Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Case managers can assist with the following services:
  • Provide resources for basic needs such as; housing, bills, food, clothing, transportation, child care and employment.
  • Assist and support clients with their specific individual needs.
  • Provide resources and assist Behavioral Health providers with client focused care requests.
  • Coordinate care for substance use disorder in-patient treatment and residential care, behavioral health in-patient and residential care, alternative learning environments, special programming and activities.
  • Meet with individuals to assist with the Behavioral Health client intake process.
  • Assists with Attention Deficit Disorder/Attention-Deficit Hyperactivity Disorder (ADD/ADHD) testing.
