Assessment, care and service coordination for older adults related to aging, memory, mental and physical health You may look for these services if you have memory loss, dementia or other aging related issues. MORE INFORMATION Cognitive assessments:

• Complete a series of standardized cognitive assessments
• Recommendations for safety around the home, medication and financial management
• Driving recommendations
• Implement compensatory strategies
• Recommendations for level of supervision or services

All evaluations are completed by licensed physical and occupational therapists. Whenever possible, services are provided in the client's home.
