Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION This service is available to all persons with Developmental Disabilities (DD). The person must have substantial functional limitations in areas such as self care, learning, mobility, communication, self direction, and independent living.
