Housing for people with low income, who are homeless or at risk of homelessness, that includes coordination of other services for residents. The coordination professional can help residents identify resources, navigate systems or help with applying to additional programs. MORE INFORMATION Maxfield Place is a permanent, supportive housing program for adults with a diagnosed disability who are considered chronically homeless. There are 10 individual apartments for single adults which include a bed, full kitchen and bathroom. Residents cook and prepare their own meals and do their own laundry. Maxfield Place staff help residents develop a long-term plan and collaborate with other service providers for referrals.
