Description
This work group was charged with identifying opportunities to improve quality of care, lower costs, and provide seamless services for Minnesotans as they move between systems of health care, long term care, mental health, dental and social services.
The work group's recommendations were included in the Roadmap to a Healthier Minnesota , which was endorsed by the Minnesota Health Care Reform Task Force on December 13, 2012. The Care Integration and Payment Reform work group's recommendations can be found in the Roadmap Supplement.
