In certain situations, the Board issues replacement licenses and public health nurse registration certificates. Please read the following information; then, download and return the Request for Document replacement form (see related content at the bottom of the page) if necessary.
All replacement documents bear the word "Replacement" and include the date that the replacement document was issued.
If your original license document has been lost, stolen, or destroyed, you are required to report the loss to the Board. You may request a replacement license by submitting the request for document replacement form along with a fee of $20.00.
Replacement Public Health Nurse Certificates
If you wish to receive a replacement public health nurse registration certificate, submit the request for document replacement form along with a fee of $5.00. Include name change documentation materials if necessary.
The Board does not issue replacement permits.
